You play as Aidan Lynch from a first-person view. After his girlfriend Sunny is taken by the Cenobites, Aidan follows her into a world that shifts between reality and the Cenobites' labyrinth, facing their cult, the Scarlet Church.

Combat mixes melee weapons and firearms. Blunt weapons break armor, sharp ones work best on unprotected enemies, and all melee weapons wear out. Some enemies cannot be beaten, so you must sneak or run.

The Genesis Configuration puzzle box is both a key to the story and a weapon, granting powers such as telekinesis. You also solve puzzles and craft healing items and ammunition from gathered resources. Doug Bradley, the original Pinhead from the films, returns to the role.

Classic Cenobites such as Chatterer, Butterball, and Deepthroat appear alongside new designs, and puzzles tied to the box and the labyrinth break up the combat. Enemies include human cultists as well as Cenobites, and a sadistic artist named Bruno Keller is among the threats you meet. Supplies are limited, so managing resources is as important as fighting.

It is aimed at adult horror fans who want a story-driven, gory survival experience.
